Marshfield Clinic Health System (MCHS) is an integrated health system serving Wisconsin and Michigan's Upper Peninsula with 11 hospitals, 65 clinical locations, more than 12,000 employees and 1,600 providers comprising over 170 specialties, Security Health Plan (SHP), Marshfield Clinic Research Institute (MCRI), and education programs.  

The mission of MCHS is to enrich lives through accessible, affordable compassionate health care. MCHS provides additional, public-facing support through programs and partnerships addressing health priorities in our communities such as alcohol and drug misuse, behavioral health, and health equity. 

Addressing Health Disparities Program Summary 

Through our Community Impact and Social Accountability efforts, MCHS and Security Health Plan have dedicated $200,000 to drive innovative, community-led projects that address health equity, including social determinants of health (SDOH). Health Equity exists when all people, regardless of race, sex, sexual orientation, disability, socio-economic status, geographic location, or other societal constructs have fair and just access, opportunity, and resources to achieve their highest potential for health. Unfortunately, determinants of health negatively affect many communities, their people, and their ability to lead healthy lives. GOAL: Health equity is achieved when every individual has the opportunity to attain their full health potential. Barriers to that potential are removed.  

APPROACH: Social determinants of health – Impacting conditions in the places where people live, learn, work, and play that affect a wide range of health and quality of life risks and outcomes. 

METRICS: Health disparity is defined as “a particular type of health difference that is closely linked with social, economic, and/or environmental disadvantage.” 

Applicants must be in the MCHS service area, as described in the eligibility section, and may request up to $10,000 maximum for a project period of one year beginning on January 1, 2024.

Eligibility  

 Eligible applicants:  

Must have tax-exempt status under IRS section 501(c) or be local government agencies

Plan projects to occur within the MCHS service area

Applicants not eligible:  

Political campaigns, candidates, parties, or partisan activities 

Religious organizations whose proposals support theological functions 

Organizations whose mission is not consistent with that of MCHS 

Youth and adult sports leagues 

Labor or fraternal groups 

Those in direct competition with MCHS 

Funding restrictions:  

Costs not related to the stated intention of this funding opportunity 

Reimbursements of expenditures already incurred 

Projects that duplicate an existing service or are covered by other funding sources 

Capital campaigns, fundraisers, entry fees/pledges 

Awards that solely benefit an individual 

Routine and regular operational and administrative costs  

 

Application Review Process and Timeline 

September 5, 2023 – Application period opens 

October 9, 2023 - Applications due by 5:00 pm CDT via online application 

October 23, 2023 – Award announcement (Notification of funding decision via email) 

Funds will be dispersed by December 29, 2023.

 Application Instructions 

Applicants are to focus on health equity with a clearly defined underserved or marginalized population, and a workplan that demonstrates a connection to reduced disparities and improved health outcomes. 

Project Abstract. Provide a summary which identifies the project title, goal statement, the population of focus. [100 words or less] 

Population of Focus and Statement of Need. Identify and describe the geographic community and population of focus whose health and wellbeing will be impacted by the work and why the perceived need exists. [500 words or less] 

Goal and Objectives. Describe what your organization plans to accomplish with this project and the steps toward accomplishing that goal. Describe the expected program outcome(s) and how the population of focus will be impacted. Include the approximate number of individuals that will be impacted. [500 words or less] 

Work Plan of Grant Activities and Timeline. Describe the key activities and processes to accomplish the project and the approximate timeline of delivery. Note any potential barriers and how you will address them. [200 words or less] 

Capacity to Implement. Describe why your organization is poised to execute this project successfully. [100 words or less] 

Evaluation and Sustainability. Describe how you will determine your project's effectiveness and how you will use it to build towards sustainability. [200 words or less] 

Budget and Budget Justification. Provide (using an Excel spreadsheet or similar) a cost breakdown of how funds will be used to meet program goals. In a brief narrative [200 words or less], provide an overview of your project budget that depicts all direct expense line items included in the budget spreadsheet. 

 

Application Scoring Criteria 

All eligible applicants will be scored on the following criteria: 

Health disparity is clearly identified.  

The target population is well defined and demonstrates a need for this project. 

The timeline is realistic. Specific project activities are described. Proposed activities support the community and address the health equity issue. The activities are sufficient to produce the desired outcome(s). Potential barriers are identified.  

The organization has experience and/or personnel to carry out the project. 

The organization has a plan to continue the work beyond the project year. 

The budget is reasonable. 

 

Reporting 

Successful applicants will provide a mid-term progress report (July 2024) and final outcomes report (January 2025). A template will be provided at the relevant time.
