MMC-BD Community Health Grant Program


The Marshfield Medical Center- Beaver Dam (“MMC-BD”) Community Health Funding Program (“Funding Program”) is part of the hospital’s community benefit and health and well-being efforts. Community Benefits are the initiatives, activities, and investments undertaken by tax-exempt hospitals to improve health in the communities they serve. Because this type of work is best done by those who are closest to the issues, the Funding Program provides funding to local organizations who are actively working to enhance existing projects or implement new ideas (collectively referred to as “Projects”) that will improve the health of residents and the community.


In 2024, the Grant Program offers two-cycles of funding (one in spring and one in fall) to organizations to address community health priorities identified in the Community Health Implementation Strategy. in the greater Dodge County areaFunding will awarded upon selection (i.e. not as reimbursements). Funding distribution is contingent upon completion of funding application, execution and compliance with funding acknowledgement and funding agreement and fulfillment of any other Funding Program requirements.


Important Funding Program Information

  1. Applicant must be one of the following (referred to individually as “Organization” and collectively as “Organizations”):
    • 501(c)(3) non-profit organizations 
      • *Non-501(c)(3) organizations can partner with a 501(3). The partnering 501(c)(3) would be the applicant, funding recipient and be obligated to meet all requirements related to receipt of any grant funding (e.g., reporting requirements). 
    • Governmental entity (e.g. local municipality, public health department, school district)
    • Other tax-exempt health care organizations and community groups may be eligible; contact the MMC-BD Community Benefits Coordinator (see below) to discuss
  2. Applicants can request between $1,000-$10,000 per application 
    • Organization may submit applications for multiple Projects 
    • Organizations may apply for funding of Projects that have previously received funding from MMC-BD
  3. Applications must be aligned with at least one of MMC-BD's Community Health Priorities:
    • Alcohol and Substance Misuse
    • Behavioral Health (Mental Health)
    • Health Equity / Equitable Access to Community-Based Resources and Services
  4. Applications should address at least one of the following community benefit objectives:
    • Improve access to health services
    • Enhance the health of the public (at a population level)
    • Advance increased general knowledge about health
    • Relieve the burden of government to improve health
  1. Applications should: 
    • Align with policy, system and environmental (“PSE”) change strategies, which are designed to promote healthy behaviors by making healthy choices readily available and easily accessible in the community. PSE change strategies are designed with sustainability and creating new normals in mind.
    • Contribute to improvements in health equity, which means that everyone has a fair and just opportunity to be healthy. This requires removing obstacles to health such as poverty, discrimination, and their consequences, including powerlessness and lack of access to good jobs with fair pay, quality education and housing, safe environments, and health care.
  2. Recipients of funding shall: 
    • Communicate with MMC-BD Community Health staff upon selection to determine specific financial documentation needed and project reporting requirements for receipt of grant funding
    • Complete and submit a W-9, provide a tax exempt form (if applicable), and sign a funding agreement
    • Submit written reports
      • Recipients of funding will be asked to submit a detailed report (template will be provided); MMC-BD staff will contact recipient when reporting window is open. 
    • Agree to provide verbal updates to MMC-BD staff and/or Community Health Sub-Committee members as requested and upon reasonable notice allow scheduled site visits to learn more about the Project and strengthen community relationships.
  3. Program Dates
    • Submission Period: February 26, 2024-March 18, 2024 
    • Review Period: April 2024
    • Notice of Approval: late April 2024
    • Distribution of Grants: May 2024 
    • Detailed Report Due: Within 1 year post distribution of grant
